TODO legal entity name pending, operating as Headwaters Longevity, is a longevity medicine practice located in Jacksonville, Florida. This policy explains how we handle information submitted through this website, which at present is a single page for people who want to be notified when the practice opens.

Information we collect

When you complete the notification form we collect:

• Your first name
• Your email address
• Your phone number
• Whether you consented to transactional text messages
• Whether you consented to marketing text messages
• The date and time of your submission

We also collect limited technical information automatically, such as your browser type, device type, referring page, and general location derived from your IP address, through website analytics.

We do not ask for and do not want health information through this website. Please do not send us details of a medical condition, medication, or symptom through this site.

Text messaging (SMS)

Text messages are entirely optional. The two consent checkboxes on our form are unchecked by default, neither is required to submit the form, and consent to receive text messages is not a condition of receiving care or of joining our notification list.

We do not share or sell mobile phone numbers or SMS opt-in data to any third party, and we do not share it with affiliates or marketing partners under any circumstances. Consent to receive text messages from us is not shared with anyone.
