By accessing or using this website you agree to these Terms of Service. If you do not agree, please do not use the site.

Services overview

This website provides information about Headwaters Longevity, a longevity medicine practice in Jacksonville, Florida, and allows you to join a list to be notified when the practice opens and when scheduling becomes available. It does not provide medical advice, does not allow you to book an appointment, and does not create a physician and patient relationship.

SMS program description

Text messages are sent by TODO legal entity name pending. There are two separate programs and you may opt in to either, both, or neither:

• Transactional messages. Appointment reminders, scheduling confirmations, and notice of when the practice opens.
• Marketing messages. Promotional messages and service announcements.

Each program requires its own separate opt-in. Both checkboxes on our form are unchecked by default and neither is required in order to submit the form.

Text messaging terms

• Message frequency varies.
• Message and data rates may apply.
• Reply STOP at any time to opt out of text messages.
• Reply HELP for help, or contact us using the details below.
• Consent to receive text messages is not required in order to receive care or to join our notification list.
• We do not share or sell mobile phone numbers or SMS opt-in data to third parties.

Carrier liability disclaimer

Wireless carriers, including T-Mobile, AT&T, and Verizon, are not liable for delayed or undelivered messages. TODO legal entity name pending is not responsible for messages that are delayed, undelivered, or misdirected due to carrier issues, network problems, or the configuration of your device.

Age restriction

You must be at least 18 years old to use this website and to opt in to receive text messages from us.

Use of website

You agree to use this site only for lawful purposes, and not to interfere with its operation, attempt to gain unauthorised access to it, or submit information that is not yours to submit.

Accuracy of information

We work to keep the information on this site accurate, but we do not guarantee it. Opening dates and availability are estimates and may change.

Limitation of liability

To the fullest extent permitted by law, TODO legal entity name pending is not liable for any indirect, incidental, or consequential damages arising from your use of this website. Nothing on this site is medical advice, and you should not rely on it in place of care from a qualified physician. If you have a medical emergency, call 911.
